Job Description:

Provides overall direction for the community operations, clinical areas, and administrative activities of the Population Health division, including Correctional Health, ensuring strategic goals are met as part of the Parkland Health & Hospital System organization.--MINIMUM SPECIFICATIONS .Education:- Must have a Master's degree in Business, Healthcare Administration, or a related field.--Experience:- Must have ten years of ambulatory care experience to include five years of management experience.- Prefer ambulatory and/or physician practice management experience. - Prefer three years of experience working in an urban healthcare system.--Equivalent Education and/or Experience- May have an equivalent combination of education and experience to substitute for both the education and the experience requirements.--Certification/Registration/Licensure:- Must have Federal Emergency Management Administration (FEMA) course certification for IS-700.A: National Incident Management System (NIMS) an Introduction or obtain with Parkland assistance within 30 days of placement in role.- Must have Federal Emergency Management Administration (FEMA) course certification for IS-100.HCB: Introduction to the Incident Command System (ICS 100) for Healthcare/Hospitals or obtain with Parkland assistance within 30 days of placement in role.- Must have Federal Emergency Management Administration (FEMA) course certification for IS-200.HCA: Applying ICS to Healthcare Organizations (ICS 200) or obtain with Parkland assistance within 30 days of placement in role.--- Utilizes creativity and innovation in attaining desired outcomes.- Uses holistic approach and persists when solving problems.--JOB ACCOUNTABILITIES1. Oversees the development, implementation, monitoring, and revision of annual goals and objectives for Population Health that support the mission and objectives of Parkland.2. Responsible for the facilitation and coordination of Population Health leadership team. Assures through collaboration that appropriate communications are occurring. Ensures established goals are enacted to the front line through leadership groups.3. Establishes in conjunction with leadership meaningful metrics to measure progress in identified high priority areas, e.g., patient experience, clinical outcomes, and financial results. Works with the team to standardize processes and make them reliable, measurable, sustainable, and easy for staff and patients..4. Understand the demand for ambulatory services through reliable metrics and develop systems to ensure timely access to necessary care5. Understand geographic variation and ensure consistent access to care across regions within the county.6. Understand health inequities and identify strategies to address them directly and in partnership with other health care entities and non-health care related CBOs.7. Supports a population-based, patient-centered approach to health that will include traditional delivery of services and that will also include alternative models of care that are more convenient for patients as well as efficient for the system. These alternative models will include e-consults, telehealth visits, facilitated networks, and others.8. Understands social determinants of population medicine in order to improve outcomes across Population Health service areas. Uses data and trend analysis to identify improvement opportunities in areas of responsibility and create high impact action plans. Incorporates clinical best practices and quality into ongoing improvements. Implements appropriate changes to improve clinical and financial outcomes that support the overall goals of Population Health. Oversees seamless coordination of care across different care environments (e.g., . Emergency Department, inpatient, specialty, primary care, correctional health, home health care, community outreach, etc.) in order to optimize the delivery of care in the most appropriate setting.9. Oversees the development of annual operating, capital, payroll and receivables budgets for billed services that ensure Parkland has the necessary funds to carry out the goals and objectives that have been established. Assists with the development and monitors Population Health contracts and leases. Superintends grants management.10. Selects, trains, schedules, motivates, supervises, evaluates, and makes recommendations at a leadership level for disciplinary actions up to and including termination, to ensure maximum utilization of individual and group capabilities. Cultivates a culture of trust, excellence, accountability and teamwork. Promotes and enables employee engagement.11. Identifies specific geographic communities in the county in need and works with community and Parkland leaders to assess needs and plan service delivery to meet, increase and/or improve services in each community. Works with communities and community agencies to promote and increase participation in Parkland health services.12. Oversees patient experience management in Population Health including grievances and complaints. Provides exceptional patient and customer experience being mindful of patients and customers. Creates an atmosphere of respect and courtesy, listening and responding to their concerns and providing clear explanations of plans. all reinforced through standardized approaches. Modifies approach, communication and interaction to take account of sensory impairments and specific needs of all customers.13. Responsible for engineering, facilities and equipment in Population Health.14. Manages Population Health compliance including The Joint Commission accreditation, Texas Jail Commission accreditation and other accreditation agencies. Establishes, maintains and implements subject matter expertise of applicable rules, regulations, policies, procedures, laws, and guidelines that impact Population Health. Utilizes expertise to develop effective internal controls that promote adherence to applicable state/federal laws, and the program requirements of accreditation agencies and federal, state, and private health plans. Seeks advice and guidance as needed to ensure proper understanding.15. Monitors the latest developments, advancements and trends in healthcare management by attending seminars or workshops and reading professional journals. Actively participates in professional organizations. Integrates knowledge gained into current work practices.
