Job Summary
The RN Spine Care Program Coordinator is responsible for the management of spine care program participants from the pre-operative education and course of treatment through the discharge and the post-operative process. The Coordinator will use a multidisciplinary approach to design, organize, and deliver a comprehensive program that will engage community, patients, family and the health care team. The Coordinator will act in a clinical role and help to assure quality, competency, and compassionate care. The Coordinator will collaborate with all members of the health care team as it relates to the entirety of the Spine care program.-- We invite you to join our nursing community and be a part of our commitment to excellence, innovation, teamwork, and compassion.
Experience and Education
--- Minimum of Bachelor's Degree in Nursing
--- A current Basic Life Support (BLS) certification, course accredited by the American Heart Association (AHA) or The American Red Cross (ARC)
--- Minimum three (3) years' experience in orthopedic nursing required.
*Experience with Neruo Surgical Patients helpful
Job Duties
--- Will assist with ensuring the spine care program is in compliance with all regulatory, state and federal guidelines/requirements and facilitate coordinating activities to meet the objectives of special certifications such as the Center of Excellence initiatives.
--- Will assist with the development and administration of a comprehensive pre-operative education program for patients and their families undergoing surgical spine care.
--- Will assist with the coordination of scheduling the required education sessions along with the surgery dates/times for surgical spine care patients.
--- Will serve as a liaison between patients/family and health care providers including but not limited to pre-admission, or, inpatient stay and discharge destination.
--- Will coordinate and administer any additional inpatient education, discharge planning and coordination/support for surgical spine care patients/family.
--- Will answers patient questions and ensure the appropriate coordination of the patient activities are completed. Orders and schedules patient appointments with providers and ancillary services. Receives, responds to, and directs patient communication, including phone calls, emails, and other interactions. Gathers needed medical information such as records, slides, films, and ancillary tests to assist in the coordination of patient needs.
--- Will assess, plan, coordinate and execute discharge plans to assure patient/family needs are met for timely and successful discharge.
--- Actively assists department leadership in continuous process improvement which includes audits of patient care satisfaction, workflow and throughput, record review, staffing education, staff development and successful monitoring of patient reported outcome measures (PROMs) related to the spine care program.
--- Assists with ensuring the spine care program is in compliance with all regulatory, state and federal guidelines/requirements.
--- Collaborates with other organizational employees to create an environment that is conducive to retention of qualified personnel.
--- Assists with staff meetings, special projects and the development, implementation and evaluation of performance improvement activities. Communicates data and solicits input from staff regarding performance and quality improvement i.e. Patient safety, infection control. Actively participates on hospital work groups.
--- Duties performed may include one or more of the following core functions: a) Directly interacting with or caring for patients; b) Directly interacting with or caring for human-subjects research participants; c) Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records); or d) Regularly maintaining, modifying, releasing or similarly affecting human-subjects research records.
--- Performs other duties as assigned.
KNOWLEDGE, SKILLS, & ABILITIES
--- Ability to work with windows-based computer systems and necessary software applications.
--- Work requires ability to respond to and interact with all stakeholders with the PACT standards.
--- Work requires ability to devise creative alternatives and suggestions to workflow constraints.
--- Work requires a change agent attitude that includes flexibility and adaptability to perform in a high volume facility.
WORKING CONDITIONS
Work is performed primarily in a hospital or hospital clinic environment. Routine walking, climbing, lifting. Work may require sitting for long periods of time while staying fully alert;
