All positions will be assigned to our Healthcare Navigation Clinical Command Center in Forney, Texas. Our Community Health EMTs & Paramedics serve as Physician Extenders to our Managed Care Patient Populations.
Duties Include:
* Communicates by telephone with patients, caregivers, family members and other clinicians to facilitate a positive patient outcome in the out-of-hospital environment
* Utilizes protocols, algorithms, sound clinical judgment and adjunctive resources (including other clinicians) to resolve patient/family/client needs successfully via both in-bound and out-bound telephone calls
* Intervenes properly in urgent and emergent situations to ensure the best possible care for the patient
* Properly records all interactions using the provided platforms and software tools in a timely and efficient manner
* Provides exemplary customer service and displays professionalism during all contacts. Works under psychologically taxing conditions to include high-stress situations
* Ability to think outside the box and solve patient / customer service issues in non-traditional healthcare model
* Performs other duties as assigned to include general housekeeping chores and special projects
