This includes preventive care, chronic disease management, patient education, and post-discharge follow-up-tailored to medically underserved populations.Location: Dallas, TX and Surrounding CountiesSchedule: Primarily Monday-Friday with occasional weekend community eventsCore Responsibilities

• Conduct comprehensive or focused physical exams and health histories.
• Order and interpret labs, diagnostics, and screenings.
• Diagnose and manage acute and chronic conditions using evidence-based guidelines.
• Prescribe medications and therapeutic interventions.
• Educate patients and families on health conditions, treatments, and preventive care.
• Maintain thorough documentation and initiate referrals as needed.
• Perform minor in-scope procedures as credentialed.
• Collaborate with physicians and care teams to optimize patient outcomes.Skills

        • Education: Graduate of an accredited Nurse Practitioner program
        • Experience: At least 2 years in primary care preferred
        • Licensure & Certification: Certified as a Family Nurse Practitioner or Primary Care NP
        • Current Texas NP license (or eligibility)
        • BLS certification (within 30 days of hire)
