Position Summary- Under the supervision of the AVP or designee, the registered nurse provides holistic, individualized patient care in response to identified needs of the general medical/surgical patient. This position focuses primarily on the post-discharge calls made to patients to optimize their safety and quality outcomes. Additional functions to be performed in accordance with the Nurse Practice Act in a manner consistent with the mission and goals of the organization.

 

Follows patient and employee safety/security policies and protocols, including hand hygiene, use of universal precautions, use of personal protective equipment, and adherence to isolation procedures.

Ensures patient/personal safety by maintaining a safe and therapeutic environment and adhering to Safety Program and Infection Control Programs.

Supports hospital and departmental Performance Improvement Goals.

Provides and documents quality patient care consistently according to the nursing process, evidence based practice, hospital and professional standards and policy/procedures.

Provides and documents post-discharge call attempts and connections; escalates clinical needs quickly and closes all loops; escalates recognition of colleagues; advances complaints and grievances quickly and to the appropriate resource to ensure compliance with CMS grievance requirements.  Ensures patient’s record is corrected or updated as needed.  Ensures patient has opportunity to ask questions and gain clarity on care plan and requested follow-up activities and care regimen.

Promotes consistent, positive patient interactions that advance the agenda of unparalleled patient service and care excellence.

Demonstrates caring and empathy in all interactions and advocates for patient- and family-centered approach in all decisions and actions.

Demonstrates excellent communication skills, including active listening.  Ensures patients, physicians, and colleagues have a clear understanding of all communications. 

Engages in evidence-based best practices that enhance the patient experience, including SAFETY rounding, communication fundamentals/AIDET, Teach-Back, Narrating Care, etc.

Participates as an active member of the health care team and positively promotes the rest of the team to patients/families/caregivers and other colleagues.

Maintains a professional appearance and demonstrates behaviors consistent with established policies/procedures.

As warranted for role, performs hourly rounding and responds promptly to call lights and patient requests. Strives to anticipate patient needs and resolve them proactively.

Charge nurse - participation in activities that promote quality patient care and efficient functioning of unit.

Seeks opportunities to impact the growth of the organization by promoting excellence in patient experience and providing high quality care to the patients served.

Identifies and assists with opportunities for performance improvement activities on the unit and with other service areas.

Fulfills the duty and responsibility to maintain competence and to continue personal and professional growth.

Creates and implements a personal career development plan, reviewed regularly by supervisor and/or preceptor.

Demonstrates fiscal responsibility by taking ownership of time management, decreasing waste of time and/or supplies, and promoting cost effectiveness while maintaining patient safety and quality of care.

Documents with high integrity all calls and escalations; actively contributes to the productivity and efficiency reports to support alignment and allocation of resources; escalates ideas and opportunities for savings and efficiencies. 

Performs other duties as assigned.

Qualifications

Graduate of an accredited school of Professional Nursing

BSN or current enrollment in BSN Program Preferred

1 year experience in healthcare setting

BLS
